A semen analysis is a laboratory examination of a semen sample, used to assess the quantity and quality of sperm. Because male factors contribute to roughly 40-50% of infertility cases, this test is recommended for every couple investigating difficulty conceiving, not just the woman. It is inexpensive, painless and gives a fertility specialist a great deal of information from a single sample.


Why is a semen analysis done?

A doctor may recommend a semen analysis when a couple has been trying to conceive for 12 months without success (or 6 months if the female partner is over 35), when there is a known or suspected male fertility concern, before fertility treatments such as IUI, IVF or ICSI, or to check the success of a vasectomy or its reversal. The terms describe how a result differs from normal: normozoospermia means all values are normal; oligozoospermia means a low sperm count; asthenozoospermia means low motility; teratozoospermia means a high proportion of abnormally shaped sperm; oligoasthenoteratozoospermia (OAT) means low count, low motility and poor morphology together; azoospermia means no sperm in the semen; and necrozoospermia means sperm are present but mostly non-viable. A single out-of-range value does not define your fertility, because sperm production runs on a roughly 72-90 day cycle and results vary.


How to prepare for and give the sample

Abstain from ejaculation for 2-7 days before the test, not longer and not shorter. Avoid alcohol and, where possible, recent illness or fever. The sample is given by masturbation into a sterile container, usually in a private room at the lab. If collected at home, keep it at body temperature and deliver it within 30-60 minutes, without lubricants or a regular condom.
